  • Current GP or specialist referral
  • Any X-rays or recent scans
  • Blood or urine test results
  • Medicare card and private health fund details
  • DVA or pension card (if applicable)

Why do I need to bring a referral?

Please ensure you bring a referral with you on the day of your appointment.

What should I expect at my consultation?

During your consultation Dr Cozzi will take a history from you and allow time to discuss any of your current concerns. Dr Cozzi may request additional diagnostic investigations including pathology and/or radiology tests for you to have done after your consultation. Should additional investigations be required, our staff will endeavour to assist you in organising these tests on the day of your visit.

Please note that we cannot discuss any patient medical matters with any other family member without written approval by the patient. You are more than welcome to bring another family member with you at the time of your consultation.

Patients that are of a non-English speaking background with limited communication ability are encouraged to have a family member or friend present at the time of consultation. Otherwise, please advise our clinic in advance and we can assist you with an interpreter service.

What is the role of the Dr Cozzi's Practice Nurse?

If surgery and or treatment is required, our Practice Nurse will establish a relationship with you immediately following your consultation. This provides support and education throughout each step of the process from diagnosis and investigation through to treatment and post operative care.

What will my procedure cost?

This will depend on which procedure is performed. A thorough estimate of fees will be discussed with you prior to booking you in for any treatments or surgeries.

What is an Estimate of Fees?

At the time of consultation we will provide you with an estimate of  fees (an indication of likely costs) charged by this practice.

Should you require in-hospital or day surgery elective procedures our estimate of fees do not include services provided by other doctors, anaesthetists, surgical assistants, radiologists or pathologists.

Dr Cozzi’s estimate of fees are also exclusive of all costs associated with your hospital accommodation, pharmacy and physiotherapy if applicable.

If you carry private health insurance, a small deposit will be required prior to your surgery with the balance of your account requiring finalisation within 30 days of your surgery. However, if you do not carry private health insurance, the total cost of Dr Cozzi’s estimate of fees will require finalisation prior to surgery.

At the time of booking your surgery you will have a consultation with Dr Cozzi’s practice manager, at which time you will be given the opportunity to discuss any questions you may have regarding fees and financial requirements.

What if I have Private Medical Insurance?

If you have private medical insurance, it is a good idea to contact your insurer prior to your admission for the procedure.

Discussing your procedure with your health fund will provide you with the full scope of your policy at an early stage so you are clear about the procedures it covers. We recommend yo enquire about claims for investigatory measures including imaging, pathology and hospital fees.

Will I be charged a cancellation fee?

Cancelling an initial consultation may incur a cancellation fee if 24 hours notice is not given.
